Policies and Consent Forms for Your Lash Business

Having policies in place protects your time, finances, and encourages new clients to respect them as well.
11 LASH POLICIES YOU SHOULD HAVE IN PLACE AS A LASH TECHNICIAN:

Minimum Age
Clients must be 18 years of age or 16 years with guardian consent. All clients must sign a waiver form.
Deposit/Reservation Fee
A $75 deposit will be charged upon booking for any new lash set. Your entire deposit will be used towards the cost of the lash set.
Arrival
Please arrive to your appointment 5 - 10 minutes early, with no makeup and very clean lashes.
If you are late or if you do not have clean lashes, this will reduce your appointment time. This may result in an incomplete look. The charge for the appointment will remain the same.
Rescheduling
If you must cancel your appointment, please give at least 24 hours’ notice to avoid cancellation fees.
Cancellation & No Show
A $50 cancellation fee will be charged for appointments cancelled less than 24 hours before the appointment start time. You will be charged 100% of your service as a cancellation fee. All future appointments will require advanced payment for booking.
Fills
40% of the eyelash extension must remain for a fill. If you have less than 40%, a full set will need to be booked. Foreign fills are on a case by case basis. They may require more time, or may require a removal. This will be charged according to time spent.
We do not perform fills on other artist’s work
Refunds
If you are unhappy with a service, please contact us within 48 hours after your appointment to discuss your concerns. Any concerns addressed after 48 hours of your last appointment, or if you failed to follow proper aftercare may result in your being charged at full price for the service.
Extra guests & pets
No Children or Pets
Food
No food or drink in the studio.
Aftercare
Clients who fail to follow our aftercare guidelines will no longer be accepted for appointments.
Cell Phone
Must be on silent or vibrate during service.
Implementation
- Set up an automated e-mail that can be sent to any client that books a new set.
- List the policies on your website.
- Have your clients sign the document.
- Ensure that current clients are notified if you’ve updated your policies and need them to sign again.
THERE ARE 2 consent forms YOU SHOULD HAVE IN PLACE AS A LASH TECHNICIAN:
- Treatment – Informs about risks and authorizes you to provide services. The same form can be used for Models.
- Photo/Video – Explains why you are taking photos or videos, what they will be used for and where, such as on social media
Why do you need an eyelash extension consent form?
- To inform your clients about the risks involved with getting eyelash extensions
- To protect you against liability if something goes wrong during the lash service
The latter is important because with any beauty treatment, there is some side effects such as allergic reactions to glue, mild irritation and inflammation around the eye or eye infections.
